Empowering Lives through Comprehensive Case Management

At Open Door Health Center of Illinois, our Social Service Team is the backbone of our commitment to holistic care. Comprising Medical Case Managers, Non-Medical Case Managers, HOPWA specialists, Retention Specialists, and Peer Advocates, the Case Management Department stands united in its mission to provide unwavering support, valuable resources, guidance, education, and advocacy to individuals living with HIV/AIDS.

Tailored Support for Diverse Needs

Recognizing the diverse backgrounds and varied needs of individuals, the Illinois Department of Public Health Ryan White Part B Program supports multiple forms of case management. Our Ryan White Case Managers are adept at identifying client needs and barriers to care, utilizing standardized assessments to guide clients into appropriate levels of activities. This includes both Non-Medical Case Management and traditional Medical Case Management, as well as collaboration with the Department of Rehabilitation Services (DRS).

From engagement and re-engagement strategies to linkage with medical and supportive services, our team is committed to promoting retention in care, ensuring medical adherence, and motivating clients towards self-sufficiency through empowerment.
